Register with us as a New Patient

Catchment area

We are happy to accept new patients who are ordinarily resident in the UK and living within our practice boundary.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

Although you will be registering with the practice you will be able to see the person of your choice – you can tell us about this at any time.

  1. If you need to be seen at short notice, and the person of your choice is not available, then you may see someone else
  2. When you come to register please bring your medical card with you together with full details of your previous GP
  3. Please tell us if you change your name or address
  4. Please register your new baby immediately

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

We have the right to refuse to register patients if they have a violent flag against their record.

Step 1: Complete online registration form

Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

Please complete one form for each family member.

Step 2: Upload scans of photo identity and proof of address

You have two options to complete your registration:

1: Deliver your completed registration form in person to one of our 3 Receptions: Kings Road, Jennyfield or Killinghall.  If you would also like online access to your medical records then please also bring with you two forms of identification.

2: Upload your completed registration form using the link below, again if you would also like online access to your medical record then please also and scan and upload photo identity and proof of address

One form of identity must contain a photo of yourself and the other must provide evidence of your current address.

Organ Donation

From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.
